What these studies investigated

Choosing a method to record jaw position

In the 2020 review, we compared clinical and instrumental methods of determining jaw position: bimanual manipulation, anterior deprogrammers, functionography, TENS, kinesiography and axiography. We also considered MRI and CBCT. The comparison covered the purpose of each method, equipment requirements and use at different treatment stages.

The practical aim was to help clinicians select methods according to the patient’s condition and the assessment goals. We considered individual movement recordings, follow-up during treatment and the use of axiographic data to set an articulator. In my practice, these recordings help establish jaw position and movement before an appliance or restoration is made.

Digital fabrication of an individual diagnostic appliance

In the 2021 study, traditional and digital fabrication of a functionograph were compared in 10 patients. This intraoral device uses a pin and plate to trace lower-jaw movements. Each patient received two devices made by different methods. The digital workflow included model scanning, design in ExoCad and 3D printing.

The results table reported fabrication times of 90 minutes for the traditional method and 30 minutes for the digital variants. Digital design allowed the insertion path, undercuts and appliance thickness to be considered. No statistically significant differences were found between the methods in the condylar-position measurements assessed by CBCT.

Within the reported protocol, digital fabrication reduced production time and allowed the shape and retention of the device to be planned in advance. These findings inform how the dentist and laboratory work together when preparing a diagnostic appliance.

Recording jaw position: comparing two methods

The 2021 study included 40 participants: 20 patients in the study group and 20 participants without signs of muscle and joint dysfunction in the control group. In the study group, jaw position was recorded using two approaches: transcutaneous electrical nerve stimulation (TENS), and TENS combined with kinesiography, an instrumental recording of jaw movement.

The recorded positions were compared using cone beam computed tomography (CBCT). Based on their analysis, the authors concluded that adding kinesiographic monitoring improved recording accuracy within the studied protocol. This monitoring allows jaw position to be observed while the record is being taken.

The method allows the clinician to monitor jaw position while taking the record used to make an occlusal appliance.

Selecting a method in the context of general health

The work by Dubova, Prisyazhnykh and Kharchenko addressed the selection of a jaw-position recording method in the context of the patient’s health. Thirty patients were examined; 15 met the study-group criteria. Functionography, transcutaneous electrical nerve stimulation and stimulation combined with kinesiography were compared, with the recorded positions assessed using CBCT.

The comparison identified differences in the condylar positions obtained with the studied methods. Together with a review of indications and contraindications, these findings informed the authors’ recommendations for selecting a jaw-position recording method.
